Personally I like the ImageShack Toolbar but it can be done manually.
http://toolbar.imageshack.us/

Pictures should not be larger than 800x600

To upload manually
go to http://imageshack.us/
Click Browse
Select File
Click open
Click Upload now
After upload
Find Forum Code
Copy then paste into your message
You can sometimes leave it all.
Or
You can cut out everything but what is between the first set of leave this part [img].
Be sure to hit preview as sometimes the picture does not show up.
OR
You can copy the direct link, paste it into the message, highlight it and hit the Img button.

Good idea posting this Frost! Another downside to phpBB is you can't attach files. Personally I've had problems with ImageShack ie pics don't show up or slow server and such however if it works for ya good. Might i suggest another:
Photobucket http://photobucket.com/ is also a good hosting site. You can tailor your pics and it has the code to copy and paste wherever you want to put it.

Another positive opinion for Photobucket, works well for me on other forums. Same deal copy and paste [IMG} code.

I think this is the better version I must use since we will just copy and paste the code.

I like photobucket because I can upload right from my phone, but a recent update seems to have removed my ability to set the size (pixels). Hmmm....

The easiest one I found was:

www.postimg.org

Dirt simple and you can do whatever size you want. I've noticed photobucket seems to only do smaller pics now on another Forum I'm on that uses them a lot. Then again, they can only do links there- I guess posting direct images with them MUST be different.
